Step 1: Register on the Patient Portal

To register you (or your child) in our secure patient portal, we’ll need your name, date of birth, and email address. After that, you’ll receive an email from TherapyNotes with a link to activate your account and complete your profile.

If you don’t see the email, please check your spam or junk folder. If it’s not there—or if the link has expired—just let us know, and we’ll send you a new one right away.

Step 2: Complete the New Patient Paperwork

Before you can schedule your first appointment, the New Patient Packet must be completed, submitted and accepted by our office. Please complete ALL the pages of the packet by clicking the link provided by TherapyNotes.

We strongly encourage these forms to be filled out using a desktop computer, laptop computer, or a tablet for ease of use, as the forms may not be mobile friendly. 

The New Patient Packet includes: 

– Client History Form

– Client Information Form 

– Client Insurance Form, including clear photos of the front AND back of your insurance cards

– Consent for Services 

– Emergency & Other Contacts Form

–  Notice & Consent Regarding Use of AI Transcription Technology 

– Notice of Privacy Practices

–  Payment Authorization Form (not required for patients who have Husky/Medicaid insurance)

– Release of Information for Emergency Contact

– Photo Identification Request, including a clear photo of your driver's license (or other current photo ID). For minors, a school ID or a recent headshot photo (such as a yearbook or school portrait) is acceptable.

*Failure to complete the paperwork in full will delay scheduling an appointment. 

Step 3: Scheduling an Intake Appointment

After all required New Patient Forms and documents (Steps 1 and 2) have been submitted and approved, our office will reach out to schedule your first appointment. You can also log into the patient portal to view available openings and request a date and time that works best for you. 

Step 4: The Day of your Intake appointment

For In-person: 

Our address is 15R Hartford Ave, Suite CD, Granby, Connecticut. The building is brick & tan. Enter at the glass double doors and proceed to the second floor. If you take the elevator, turn right. If you take the stairs, turn left. We are at the end of the hallway. Please come in and take a seat in the waiting room. We will be with you shortly. 

For Telehealth:

Telehealth sessions allow us to meet while you are in the comfort and convenience of your own home. Any device with a camera, microphone, compatible web browser, and Internet access is suitable for a telehealth session.

To Join a Telehealth Session

Navigate to the Telehealth tab on the Healthru Therapy portal either by using the link in your appointment reminder or this link.

Enter your clinician’s name in the drop down to select their waiting room and click Join Waiting Room to be connected to their waiting room.

Your browser may prompt you to share access to your camera and microphone. Click Allow.

On the telehealth welcome dialog enter your name, and if the session is for another user you may enter their name under “Client’s Name.” Select the Camera and Microphone you want to use for your session. You should see a preview of your video and an audio meter that responds to your voice.

When you're ready to join your session, click the Join Session button.

Before you join the session, we’ll take your picture to let the clinician know you’re ready. This photo will not be stored after your session.

Once you indicate that you're ready for your session, the clinician will get a notification and can accept you out of the waiting room when they’re ready.

In a telehealth session, your video will appear in the upper left corner of your browser, and the provider's video will appear larger in the middle of the browser. You can disable your camera, mute your microphone, share your screen (if applicable) and end the session using the buttons in your control tray.

When a session ends, you'll be taken back to your home page. If you end the session by mistake, you may rejoin the session. To rejoin a session, enter the clinician’s name in the field and select Join Waiting Room to be admitted into their waiting room again.
